POSITION SUMMARY:
Serve as the day-to-day lead for the Joint Program and supervise the development, implementation, and organization of the Joint Program. The Care Coordinator (CC) coordinates the efforts between surgeons, managers and staff while tracking the progress of the program and identifying areas requiring attention. The CC is responsible for all program management, report generation and review, process improvement, patient satisfaction, facilities review, budget and administrative activities within the Program.

PRIMARY JOB RESPONSIBLITIES:
• Lead regularly scheduled meetings with members of the management team, the unit medical director, other physicians and key staff.
• Manage the build out and oversee the day-to-day operations within the Joint Program.
• Develop standardized care plans with the surgeons including: pre-op, post-op and discharge orders.
• Ensure appropriate pre-op preparation for each patient prior to surgery.
• Develop and oversee the creation of pre and post-op teaching classes including the coordination of teaching materials with the physicians’ office.
• Create measurement system or use current date for tracking surgical outcomes and patient satisfaction, identifying areas that require improvement.
• Provide key statistics at Leadership Team meetings and to other stakeholders as necessary. Statistics may include: outcomes, progress against budget, patient satisfaction, complications, denied days, hospital-wide survey results, PROMs data, nurse call back results, etc.
• Develop nursing education handouts/online education.
• Develop a community education and seminar program inclusive of schedules, presenters, content and materials.
• Create and oversee an ongoing quality improvement plan.
• Identify patient amenities and secure necessary resources to administer the services.

QUALIFICATIONS/EXPERIENCE:
• Minimum of 5 years of experience, Orthopedic experience preferred.
• Able to complete and/or experience with all job responsibilities.
• Able to communicate with patients, family and staff in a professional, friendly manner.
• Proven working relationship with surgeons.
• Experience with patient / staff education.
• Proven leadership experience, having managed resources either directly or within the implementation of a new initiative.
• Strong detail-orientation and follow-up capacity.
• Excellent organizational skills.
• Ability to multitask.
• Demonstrated success in relationship building.
• Ability to keep team focused on goals / timelines.
• Customer service focused.
• Strong data analysis skills, requiring interpretation of clinical results to develop program improvement projects.
• Experience in achieving results and working with process management.

EDUCATION:
Clinical background – R.N., P.T., PA-C, APNP, preferred.
Bachelors degree required.

LICENSES/CERTIFICATIONS:
Licensed in State of Wisconsin in clinical practice area or eligible for licensure.

Job Summary:

Provides services as a patient navigator throughout the continuum of care.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
  • Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
  • Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.
  • Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.
  • Establishes working relationships teaching protocols for patient education, family counseling and general public information.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

EDUCATION

  • Graduate of accredited school of nursing or education equivalency for licensing

EXPERIENCE

  • Three years' registered nurse experience


PHYSICAL REQUIREMENTS

  • Constant use of speech to share information through oral communication.
  • Constant standing and walking.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, reaching and keyboard use/data entry.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of smell to detect/recognize odors.
  • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Occasional lifting/moving of patients.
  • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
  • Occasional driving.
  • Rare crawling and running.
