Closed-Door Long-Term Care (LTC) Pharmacist

The Pharmacist performs professional pharmaceutical work in a closed-door long-term care pharmacy serving residents of skilled nursing facilities, assisted living facilities, and other long-term care settings. Responsibilities include reviewing and verifying medication orders for accuracy and appropriateness, overseeing dispensing operations, ensuring compliance with federal and state regulations, and collaborating with prescribers, nurses, and pharmacy staff to support safe and effective medication therapy.

The Pharmacist is responsible for identifying potential drug interactions, duplicate therapies, dosage concerns, and adverse drug events while ensuring timely delivery of medications to LTC residents. The Pharmacist also provides drug information and clinical guidance to facility staff, supports medication regimen reviews, monitors prescribing trends, and helps maintain compliance with long-term care pharmacy standards and quality initiatives.

Minimum Requirements:

  • Current registration and/or license as a Pharmacist in the state of practice required
  • One (1) year of pharmacist experience preferred; long-term care, closed-door, institutional, or consultant pharmacy experience preferred
  • Knowledge of long-term care pharmacy operations, medication packaging systems, and regulatory requirements
  • Ability to review, verify, and dispense prescriptions accurately in a high-volume LTC pharmacy environment
  • Strong understanding of medication therapy management, drug interactions, and geriatric pharmacotherapy
  • Complies with all applicable professional standards of practice
  • Current CPR certification if required
  • TB questionnaire, PPD, or chest x-ray if required
  • Current health certificate (per contract or state regulation)
  • Must meet all federal, state, and local requirements
  • Must be at least 18 years of age

This version reads much more like a true closed-door LTC pharmacist role and removes references to IV compounding and direct patient counseling that are more common in hospital or infusion settings. It emphasizes order verification, nursing facility support, regulatory compliance, medication reviews, and geriatric populations, which are the core functions LTC pharmacists expect to hear.
