Salary: $109,000 – $135,000 + incentive bonus

Bonus: Upto $10,000 Sign-On | Relocation (case-by-case)
Shift: Full-Time, Days

Job Summary

Lead hospital-wide case management operations, ensuring efficient care coordination, discharge planning, and optimal patient outcomes.

Key Requirements

  • RN / LCSW / LMSW license
  • 3+ years acute care case management
  • 2+ years leadership experience
  • Strong skills in LOS reduction & process improvement
