ESSENTIAL DUTIES:
Model a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients, visitors, and co-workers- Ensure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
- Maintain good attendance record
- Provide resident therapy treatments, evaluations, recertifications and discharges
- Document treatment and progress notes and other clinical work in the medical record per departmental policy and procedure
- Provide resident, family and staff education pertaining to Physical Therapy modalities to ensure understanding and proficiency of care
- Demonstrates good clinical judgment in all aspects of resident treatments in which trained to provide
- Screening residents to determine their potential benefit from physical therapy intervention and communicate need for intervention
- Develops, directs and/or participates in physical therapy programming for the residents
- Attends in-services and other meetings as required on an ongoing basis as needed
- Maintains continuing education per state licensure requirements
- Other duties as assigned by the Director of Rehab
MINIMUM QUALIFICATIONS:
Good standing Physical Therapy license in the state of Texas- Knowledge of therapy industry and federal, state and local government regulations
- Preferred experience with NetHealth documentation system
- Demonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment
- Demonstrated efficiency strategies for fast paced environment
- Experience in SNF/LTC/IL/AL/OP setting or mature population rehabilitation and wellness needs
- Experience with PDPM treatment and reimbursement policy
- Basic computer skills, Outlook, Word, Excel
- Excellent customer service skills
- Genuine caring for and interest in the care of mature adults, persons with a handicap and hospice patients
- Ability to perform work tasks within the Physical Demand Requirements
- Ability to comply with organizational and departmental safety policies and procedures
