Job Summary:
As a Physical Therapist with Freedom At Home, you will provide professional services within the scope of physical therapy practice standards, collaborating with primary care physicians and other members of the healthcare team to deliver exceptional, client-centered care. You will teach and educate patients and their families, and work to foster independence and promote a fulfilling life.
Responsibilities:
Receives intake referral information and prepares PT paperwork/tools necessary for visits
Performs PT assessments, documents accordingly, and helps develop an effective PT Plan of Care (POC)
Evaluates patient function, mobility, and independence, and develops strategies to promote improvement
Communicates effectively with patients, families, and healthcare teams to ensure seamless care
Demonstrates expertise in prosthetic, orthotic, and assistive devices, and educates patients on safe use
Maintains accurate and timely clinical records, and participates in peer review and performance improvement initiatives
Participates in staff meetings, in-services, and growth opportunities to enhance practice and patient care
Qualifications:
Successful completion of a PT program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations
Current, active license to practice Physical Therapy in Michigan
Minimum of two (2) years' experience as a Physical Therapist
Criminal background check and Current CPR certification
Ability to meet the physical demands of the job, including lifting, transferring, and standing for extended periods
Work Environment:
Indoors, performing patient assessments and tasks in patient residences
Travel to patient residences and meetings, with possible exposure to communicable diseases
Functional Abilities:
Ability to access patient homes that may not be handicap accessible
Ability to hear, speak, understand, and communicate effectively in English
