Join Autumn Lake at Greenfield as a PRN Physical Therapist (PT) and provide individualized rehabilitation services in a collaborative skilled nursing and post-acute care environment.
Position Responsibilities
- Perform physical therapy evaluations and establish individualized plans of care
- Provide skilled interventions focused on strength, balance, gait, mobility, and functional performance
- Monitor resident outcomes and modify treatment plans as appropriate
- Educate residents and caregivers on mobility and safety strategies
- Work closely with PTAs, nursing, and other rehabilitation professionals
- Maintain timely clinical documentation
Qualifications
- Active Wisconsin Physical Therapist license required
- Strong clinical assessment and communication skills
- Skilled nursing or post-acute care experience is a plus
Flexible PRN hours • $55–$65/hour • Collaborative therapy environment
