Position Summary: The Family Support Services Supervisor is responsible for coordinating allaspects of patient social services(social work, chaplaincy, & bereavement) as well as the area of volunteer coordination and community outreach.This position is responsible for establishing, implementing, and evaluating goals and objectives tomeet and promote the standards of quality and contribute to the PVHPC missionand philosophy.

Essential Job Functions

  • Superviseandcoordinatethe Family SupportServices Departmentunderdirectoversight of theExecutive Director. The Family Support ServicesteamconsistsoftheVolunteer Coordinator, Bereavement Coordinator, Chaplains,Social Workers,andIntegrative Therapists.
  • Develop and implementtheobjectives, policies, procedures,andjob descriptions fortheFamily Supports Services Department.
  • Ensure thatallFamily Support Services are delivered consistently with theplan of careandin accordance withstandards of practice.
  • Ensure compliance with all state, federal,and CHAP regulatory requirements.
  • Coordinatethe implementation of improved work methods and procedures to ensure services are deliveredin accordancewithpolicy.
  • Ensure seamless careservice delivery by providing oversight ofthecoordination of care with all clinical service providers.
  • Assistinrecruiting,selection, orientation, evaluation,and directlymanagemembers of the Family Support Services Team.
  • Coordinateand supervise the assessment, planning, implementation,and evaluation ofthe clinicalcare by members of the Family Support Team.
  • Assure proper documentation in clinical recordscomplieswithstate, federal,and CHAP standards.
  • Inconjunction withtheExecutiveDirector,provideleadership in strategic planning, including identifying opportunities foradditionalor improved services to address unmet customer needs.
  • Maintainacomprehensive working knowledge of the field of social work andshareinformation withappropriate organizationalpersonnel.
  • Maintain knowledge ofPVHPCcontractual relationships andensureFamily Support Services are delivered according to contractual provisions.
  • Maintaincomprehensiveknowledge of community resources andassistreferral sources in accessing community resources should services not be provided byPVHPC.
  • Assistpatients and families with practical needs related to orexacerbatedby the terminal illness by helping themutilizecommunity resources if needed.
  • In collaboration with the Clinical Educator,Plan and implementin-serviceand continuing education programs to meet the development needs of the Family Support Services Team.
  • Attend and activelyparticipatein weekly IDT meetings.
  • Participate in thePVHPCquality improvement process.
  • Monitor Family Support Services caseload,assignstaff to new cases,reviewdocumentation for staff training and quality,maintainstaff/caseload ratio,and ensurea timelyresponse in meeting referral and client needs.
  • Coordinateanddevelopcommunity outreach services and presentations.
  • Administrator on-call responsibilities as needed.
  • Maintains a manageable caseload
  • Other duties asassigned.

Qualifications

(Minimum qualifications will be considered required unless specificallystatedotherwise)

Experience:

  • Minimum oftwo(2) yearsofexperience in health care.
  • Experience in hospice careis preferred.
  • Understands hospice philosophy and issues of death/dying.

Education and Training:

  • A graduateof amaster’sprogram in social work accredited by the Council on
    Social Work Education.

License, Certification & Registration:

  • Possesses andmaintainscurrent CPR Certification.
  • Current Massachusetts licensure: LICSW
  • Certification in a specialty field, such as family counseling, death and dying,preferred.
  • Valid driver’s license

Other Requirements:

  • Demonstrates good verbal and written communication andorganizationskills.
  • Ability to deal tactfully with patients, families,andthecommunity.
  • Knowledge of corporate business management, governmental regulations,CHAP standards, local and state resources,assistanceprograms,and Medicare/Medicaidpreferred.
  • Demonstrates autonomy, organization, assertiveness, flexibility,and cooperation in performing job responsibilities.
  • Demonstrated skills in family counseling and/or group facilitation.
  • Ability tosolve problems, delegate responsibility,and encouragehigh-levelperformance fromtheFamily Support Team.
  • Ability to understand and implement quality improvement processes.
