Nurse Care Manager

The Care Manager is responsible for managing the clinical, medical, and psychosocial care for clients, including vulnerable adults and elderly populations. Frequent local travel is required, intermittently throughout the day, depending on the client’s needs.

The Option Group is a small Care Management organization who is celebrating 15 years in business! We are looking for a Nurse Care Manager who has a nursing background and care management/care coordination experience.
