Nurse RN LPN

This nursing role focuses on delivering private duty care in home settings, providing personalized one-to-one nursing support. The RN or LPN performs essential clinical tasks, including G-tube feeding, medication administration, tracheostomy care, and accurate patient documentation in the Electronic Health Record (EHR). This position involves local travel to patient homes to ensure comprehensive care and monitoring.

Responsibilities

• Provide private duty nursing care in home settings
• Administer G-tube feeding and medications
• Perform tracheostomy care and monitor vital signs
• Conduct patient assessments and develop care plans
• Maintain accurate patient documentation using EHR systems
• Implement infection control procedures
• Offer patient support and communication
• Respond to emergencies as needed
• Travel locally to patient residences

Required Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) diploma

Preferred Qualifications

• 1+ years of nursing care experience
• Valid LPN or RN license
• Experience with G-tube feeding, medication administration, and tracheostomy care
• Proficient in patient documentation and vital signs monitoring
• Knowledge of infection control practices
• Strong patient communication skills
• Emergency response capability
