This role provides comprehensive care management and social work case management services to elderly individuals and adults with physical and intellectual disabilities, helping them live as independently as possible in the community. The Care Manager collaborates closely with an RN Care Manager and an interdisciplinary team to assess member needs, develop and implement member-centered care plans, coordinate services, and ensure quality and continuity of care. This position offers a weekday schedule and the opportunity to contribute meaningfully within a mission-driven program focused on community-based support.
ResponsibilitiesThis position is based in an office setting in Rock County, Wisconsin, with the first day of orientation conducted at a Vliet location. The standard work schedule is Monday through Friday, 8:00 a.m. to 4:00 p.m., providing a consistent weekday routine. The role involves regular travel within Rock County to meet with members, families, and community partners, requiring a valid driver’s license, a personal vehicle, and appropriate insurance coverage. Work is performed in a professional healthcare and social services environment that emphasizes team collaboration, use of healthcare management systems, and adherence to documentation and quality standards. Job Type & LocationThis is a Contract to Hire position based out of Elkhorn, WI.
Pay and BenefitsThe pay range for this position is $25.00 - $35.00/hr.
Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
