Job Title: Care Manager / Case ManagerJob Description

This role provides comprehensive care management and social work case management services to elderly individuals and adults with physical and intellectual disabilities, helping them live as independently as possible in the community. The Care Manager collaborates closely with an RN Care Manager and an interdisciplinary team to assess member needs, develop and implement member-centered care plans, coordinate services, and ensure quality and continuity of care. This position offers a weekday schedule and the opportunity to contribute meaningfully within a mission-driven program focused on community-based support.

Responsibilities
  • Manage an assigned caseload of program members in collaboration with an RN Care Manager.
  • Complete comprehensive member assessments, including physical, psychosocial, and functional evaluations.
  • Develop, implement, and regularly update member-centered care plans based on assessed needs and goals.
  • Conduct functional screens and ensure accurate, timely completion of all required assessments.
  • Provide ongoing care coordination for members, ensuring services are in place, appropriate, and responsive to changing needs.
  • Monitor the quality and effectiveness of services and supports, and adjust care plans as needed.
  • Provide direct social work case management services, including advocacy, resource linkage, and support to members and their support systems.
  • Facilitate team-based care management, participating in interdisciplinary meetings and collaborating with colleagues to coordinate member care.
  • Maintain required documentation and member-related paperwork in adherence to established standards, policies, and regulatory requirements.
  • Use healthcare management systems to document services, track member progress, and support care coordination.
  • Ensure compliance with applicable healthcare programs such as Medicare, Medicaid, managed care, and related benefit programs.
  • Engage members, families, and caregivers in the care planning process, promoting autonomy, dignity, and person-centered outcomes.
  • Support members in navigating community resources, benefits, and services to maintain independence in the community.
  • Regularly travel within the local area (Rock County and surrounding service area) using a personal vehicle to visit members and community partners.
  • Maintain a valid driver’s license and required insurance coverage for work-related travel.
Essential Skills
  • Master’s degree in Social Work required; the master’s degree must be in Social Work.
  • Current Wisconsin Social Work licensure required.
  • Minimum of 1 year of experience providing care or services for a frail or elderly population required.
  • Ability to complete comprehensive assessments, member-centered care plans, and functional screens.
  • Experience providing care coordination or case management services in a healthcare or social services setting.
  • Strong understanding of Medicare, Medicaid, managed care, and related healthcare programs.
  • Proficiency in maintaining accurate, timely documentation and adhering to documentation standards.
  • Ability to work collaboratively with an RN Care Manager and an interdisciplinary team.
  • Valid driver’s license, access to a personal vehicle, and proof of required insurance coverage.
  • Strong communication, interpersonal, and advocacy skills to work effectively with older adults and individuals with physical and intellectual disabilities.
Additional Skills & Qualifications
  • Bachelor’s degree in Human Services or a closely related area required if a Master’s degree in Social Work is not held.
  • A minimum of three (3) years of experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services bachelor’s degree.
  • One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred.
  • 1 to 2 years of case management experience with elderly and/or developmentally or physically disabled populations preferred.
  • Background in social work, psychology, or human services.
  • Experience as a care manager, care coordinator, or in care management within the healthcare industry.
  • Familiarity with healthcare management systems and electronic documentation.
  • Certified Functional Screener credential or experience performing functional screens is beneficial.
  • Ability to facilitate team-based care management and collaborate in a multidisciplinary environment.
  • Commitment to mission-driven, community-based work supporting independence for elderly and disabled adults.
Work Environment

This position is based in an office setting in Rock County, Wisconsin, with the first day of orientation conducted at a Vliet location. The standard work schedule is Monday through Friday, 8:00 a.m. to 4:00 p.m., providing a consistent weekday routine. The role involves regular travel within Rock County to meet with members, families, and community partners, requiring a valid driver’s license, a personal vehicle, and appropriate insurance coverage. Work is performed in a professional healthcare and social services environment that emphasizes team collaboration, use of healthcare management systems, and adherence to documentation and quality standards. Job Type & Location

This is a Contract to Hire position based out of Elkhorn, WI.

Pay and Benefits

The pay range for this position is $25.00 - $35.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
