SYNERGY HomeCare of Central Illinois is looking for a compassionate, organized, and relationship-driven Client Care Manager (CCM) to join our team!

The Client Care Manager plays an important role in both our client experience and company growth, helping guide clients from their first inquiry through onboarding and ongoing care. You'll build relationships with clients and families, conduct assessments, solve problems, and work closely with caregivers and the office team to make sure our clients receive the best possible experience.

This position also works alongside the Office Manager to help lead the day-to-day operations of the office. Our ideal CCM is someone who enjoys people, stays organized, takes ownership of their responsibilities, and isn't afraid to jump in and help when things get busy. This reflects the broader role in our current materials, which includes both client-care responsibilities and hands-on office leadership.


What You'll Do:

Lead & Intake Management

  • Manage prospective client inquiries from initial contact through onboarding

  • Provide consistent follow-up and maintain accurate documentation

  • Build relationships with prospective clients and families and help them understand their care options

Assessments & Client Onboarding

  • Conduct in-home assessments to understand each client's needs, goals, and home environment

  • Develop detailed plans of care and proposed schedules

  • Complete required paperwork and help new clients successfully transition into services

  • Follow up with new clients to make sure things are going well

Ongoing Client Care

  • Build and maintain strong relationships with clients and their families

  • Proactively address concerns before they become bigger issues

  • Monitor changes in condition and complete reassessments when needed

  • Complete quality reviews and client follow-ups

  • Review caregiver documentation and follow up on concerns or changes

  • Keep client records and documentation accurate and up to date

Caregiver & Office Collaboration

  • Work with the team to match clients with compatible caregivers

  • Help resolve caregiver-client concerns

  • Co-manage daily office operations alongside the Office Manager

  • Support supervisors and collaborate on day-to-day decisions

  • Jump in with phones, scheduling, interviews, caregiver needs, and other office priorities when needed

The CCM has always been intended to support beyond a narrow client-management lane; the formal description includes interviewing, phones, filling shifts, monitoring scheduling concerns, and helping with overall office needs.

Community & Referral Relationships

  • Develop and maintain relationships with referral partners

  • Communicate with referral sources regarding shared clients

  • Educate community partners and families about SYNERGY's services

  • Partner with our Regional Community Liaisons to support growth and community relationships

Healthcare, home care, case management, social services, sales, or other relationship-focused experience is preferred. We're also looking for the right personality and mindsetsomeone who is caring, dependable, proactive, and excited to be part of a team.

More importantly, we are looking for someone who demonstrates initiative, accountability, adaptability, professionalism, and a team-first mindset.

Benefits

  • Salary: $50,000–$55,000

  • Monday–Friday, approximately 9:00 AM–5:00 PM
