Health Navigator - Float (Job Overview)Schedule: Monday-Friday (8AM-5PM)As a Clinic Float Health Navigator, you will build meaningful relationships with community members and Legacy patients while helping them access medical care, stay engaged in treatment, and adhere to medications for HIV prevention or treatment through Legacy Pharmacy partnerships. Your compassion and guidance will help patients navigate care with greater confidence and connect with services that support their health and well-being.
As an integrated member of the clinical care team, you will serve patients in both clinic and community environments. You will help identify individuals who may benefit from prevention services and connect them with Legacy care. Current areas of focus include HIV/STI testing, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (nPEP), HIV Treatment as Prevention (TasP), medication adherence, and general safer sex practices.
This role travels between different Legacy locations to provide services where they are needed most. You will collaborate with clinical professionals, community partners, patients, and families to promote timely, culturally sensitive, language-appropriate, and continuous care. Key Responsibilities- Implement health education programming in one-on-one and group settings to increase patients’ knowledge to prevent illness, manage health conditions, and improve overall wellness.
- Use health education, navigation, disease self-management, goal setting, motivational interviewing, and other proven techniques to meet specific needs and assist patients in setting health-related goals.
- Provide information, referrals, and assistance with linkage to services with the goal of improving patients’ independence and overall health while supporting their varying routes throughout Legacy.
- Provide point-of-care testing for HIV and other health conditions in a clinical setting.
- Contribute to the development of new health education programming identified through interactions with individual, clinic, and community participants.
- Serve as a member of an interdisciplinary clinic and care team and promote effective working relationships with internal and external stakeholders.
- Conduct brief assessments of patient behavioral, social, and medical needs, readiness for health behavior change, and progress over time.
- Establish and maintain referral processes, resource lists, and other community networks to effectively link patients to needed health information and services.
- Work within the context of a primary care medical home, applying a team-based approach to care while partnering with patients and families to promote timely access to care, culturally sensitive and language-appropriate understanding of patient care, continuity of care, and whole-person improvement through appropriate self-management skills.
- Meet a high standard of customer care to ensure a positive patient experience.
- Document all patient services in the electronic health record and other data systems as required.
- Manage patient schedules and desktop in the electronic health record system.
- Meet assigned performance metrics, including those related to grant or contract requirements, if applicable.
- Travel between clinics as assigned on a routine basis.
- Perform other duties as assigned.
Minimum Qualifications- High School Diploma or equivalent
Required Years of Experience- 3 - 5 years of relevant experience
Desirable Education and/or Experience- Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred
- Experience performing point-of-care testing to clients in a clinical or community setting
- Demonstrated actions and activities with respect to HIV care and prevention
- Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
- Previous experience with Electronic Health Records Systems (EHRS)
- Ability to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire.
Required Knowledge, Skills, and/or Abilities- Excellent communication skills and customer service skills.
- Adept in decision making responsibilities.
- Working knowledge and understanding of statistics and graphical representation principles.
- Strong computer skills (MS Office). Demonstrated competency with an EHRS system
- Ability to manage multiple deadlines and prioritize tasks effectively.
- This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
- Ability to demonstrate core competencies of role within 90 days of hire
Desirable Knowledge, Skills, and/or Abilities- Bilingual English/Spanish fluency.
