Responsibilities:
Conduct MDS assessments and develop individualized care plans in collaboration with the interdisciplinary care team
Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
Provide direct care to residents, including medication administration and mobility assistance
Monitor and report changes in resident condition to the care team
Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
Maintain accurate and detailed records of resident care and assessments
Requirements:
Current RN or LPN license in the state of Illinois
Minimum of 1-3 years of experience in senior care or a related field
Strong understanding of MDS documentation and assessment protocols
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Compassionate and patient-focused approach to care
