Gurwin Healthcare System is a full healthcare continuum providing extraordinary care to the Long Island community and beyond. Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies.

DUTIES AND RESPONSIBILITIES:

  • Provide advice and counsel to all levels of management pertaining to employee relations issues and workplace concerns, including performance improvement plans e.g., documentation of steps taken and upon consultation with the Vice President and Chief People Officer, authorization of termination actions.

  • Consult with and schedule time as necessary to meet with employees who want to speak with Human Resources. Respond to employee concerns, conduct fact-finding investigations as warranted and prepare recommended courses of action for review. Offer mediation services to resolve workplace differences between employees as well as supervisors and staff. Provide guidance and support as it relates to regulations, policies and practices.

  • Complete required documentation (profiles) for our contracts for newly assigned personnel and file in employee chart. Follow up as necessary to obtain incomplete and / or missing documents.

  • Coordinate employee benefits, such as medical, dental, vision, tuition reimbursement. Respond to inquiries from staff and assist with benefits open enrollment. Audit monthly bills to ensure accuracy with billing, removing and adding employees/dependents as appropriate.

  • Administer leave of absence paperwork, such as Family Medical Leave Act (FMLA), Paid Family Leave (PFL) or personal leave. This includes providing eligibility information to employees, preparing paperwork, following up to obtain appropriate documentation, tracking of leaves and ensuring appropriate return to work documentation.

  • Responsible for the recruitment process including developing/updating job descriptions, posting available positions on Apploi, screening resumes, interviewing, reference checks, extending offers, scheduling new hire pre-employment processing, background checks, Home Care Registry (HCR) and Criminal History Record Check (CHRC) (i.e fingerprinting).

  • Primary contact for generating, routing Request for Change forms, as it relates to employee personal information and Position Requisitions Discuss issues / concerns with payroll to ensure accuracy with employees pay.

  • Process weekly payroll for LHCSA administrative/Per Diem and submit it to the payroll department. Monitor and complete SmartLinx platform for LHCSA administrative staff.

  • Responsible for conducting new hire orientation and completing all HR related paperwork.

  • Ensure monitoring including but not limited to: tracking of licenses/certifications, performance evaluations, I-9 information, and work authorizations.

  • Responsible for coordinating and promoting various employee recognition events. This includes logistics and applicable presentations for the Employee of the Month, Winter Celebration, Employee Appreciation Day, Annual Staff BBQ, Assisted Living Week and other impromptu recognition events.

  • External reporting requirements – i.e., OSHA (annual) and contracts. Responsible for sending requests to DOH concerning HHATP, maintain HHATP binder in current status by sending notices of HCR classes.

  • Stay abreast of changes and evolving interpretations of state and federal laws, as well as an understanding of Home Care regulations as it relates to changes to human resources handbook, policies, and procedures. Collaborate with the Vice President and Chief People Officer regarding the development of new or revised policies that may be warranted by regulatory changes, adoption of HR best practices or workplace trends.

  • Willingness to accept and handle other projects as assigned by his / her supervisor. Performs related tasks as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor's degree in Human Resources or related field.

  • 2-3 years’ generalist experience that includes employee relations, benefits administration, employment law and compensation.

  • Previous HR experience in a healthcare/Home Care setting strongly preferred

  • Excellent organizational skills, highly detail oriented with strong analytical skills.

  • Excellent oral and written communication skills. Must be a critical thinker.

  • Proficiency in MS Office Suite: Outlook, Word, Excel, PowerPoint

  • Deal discretely with sensitive information concerning performance of staff and Human Resources issues.
