We are seeking a compassionate, relationship-driven Social Services Coordinator to help residents and their families navigate life's transitions with dignity, empathy, and confidence.

What You'll Do
As the Social Services Coordinator, you will:
- Build meaningful relationships with residents and their families.
- Assess residents' social, wellness, and support needs.
- Coordinate transitions throughout our continuum of care, including Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Rehabilitation.
- Connect residents with internal and external community resources.
- Advocate for residents while serving as a liaison between residents, families, physicians, and community departments.
- Monitor resident wellness and proactively identify opportunities for additional support.
- Coordinate health education programs, support groups, and wellness initiatives.
- Participate in interdisciplinary meetings to promote resident-centered care.
- Help create a welcoming experience for new residents through orientation and ongoing support.
- Promote resident engagement and overall well-being through collaboration and compassionate service.

Qualifications
- Bachelor's degree in Social Work or a related field required.
- Experience in senior living, healthcare, case management, social services, or care coordination preferred.
- Strong interpersonal, organizational, and communication skills.
- Ability to maintain confidentiality and exercise sound judgment.
- Ability to manage multiple priorities in a fast-paced environment.
