RN Care Manager (RN Supervisor)

allaround Concierge Care is a private-pay, non-medical home care agency operating in Miami-Dade and Monroe Counties under AHCA licensure. We are a technology-integrated, W-2 employer running on a connected platform with fully digital and online scheduling, EVV, clinical documentation, HR, payroll, training, and AI-assisted caregiver-client matching.

Our systems include AxisCare for scheduling, EVV, care plans, and family-facing dashboards; Nevvon for structured training and compliance; Vivantium for HR and payroll; and a proprietary AI agent layer that automates intake, reporting, and operational handoffs. Although we are a non-medical agency, clinical-grade standards are built into our infrastructure.

The RN Care Manager is a field-based leadership role responsible for in-home client assessments, individualized care plan development, and direct supervision of W-2 Care Professionals. This position is a core member of the care management team, reporting directly to the Administrator and CEO.

• Conduct in-home assessments at client intake and on an ongoing basis

• Develop, document, and maintain individualized care plans in AxisCare

• Provide field-based supervision of W-2 Care Professionals, including first-shift presence for all new client placements

• Conduct 30-day supervisory visits for all active clients

• Monitor care quality and respond to care concerns from clients and families

• Maintain AHCA compliance documentation and support regulatory readiness

• Collaborate with the Care Coordinator on scheduling, client-caregiver matching, and shift continuity

• Active Florida Registered Nurse (RN) license in good standing

• Minimum 3 years of experience in an RN supervisory or management capacity

• Home care, home health, hospice, or acute care with field-based supervision experience preferred

• Proficiency with EMR and scheduling platforms; AxisCare experience is a plus

• Strong clinical documentation skills

• Reliable transportation for field visits throughout Miami-Dade and Monroe Counties

• Bilingual English and Spanish strongly preferred

• W-2 direct employment

• $70,000 to $90,000 annually, commensurate with experience

• Paid vacation: 1 week after 6 months; 2 weeks after 1 year of full-time employment
