Summary

The Client Care Coordinator will be responsible for overseeing the delivery of

high-quality home care services across multiple counties and various contracts.

This role is pivotal in ensuring that clients receive tailored care plans, facilitate

communication between clients, families, and healthcare providers, and manage

the logistics of service delivery.

Job Responsibilities

• New Client Intake Responsibilities

o Inputs all new clients into AxisCare, ensures billing entered, updates annual

service agreement dates to ensure no lapse in services.

o Process incoming client referrals and document new client information in

AxisCare.

o Collaborates with Scheduler, HR, and DOO to discuss rising care needs and

conflict resolutions.

o Completes customer service “Rising Star Surveys,” follows up with new clients to

ensure satisfaction, provides

• Coordination of Service

o Manage client scheduling activities and notify clients of changes, call-offs, or

relevant updates.

o Coordinates Service plan needs with Scheduler

o Liaise with healthcare providers, family members, and clients to facilitate

communication and address concerns.

• Contract Management

o Oversee compliance with various contractual obligations, ensuring services meet

the standards set by funding sources

o Monitor documentation to ensure compliance with clock-ins, clock-outs, care

documentation, and signatures.

o Over/under/missed visit reporting

o Oversee call-off documentation• Community Outreach and Networking

o Build and maintain relationships with community organizations and healthcare

providers.

o Networking to local hospitals, independent living facilities, long term care

facilities, independent specialty provider offices, and other networking events.

o Represent agencies in community events and meetings to promote services and

gather feedback.

• Operational Support

o Answer phone calls using the “One Talk” system and other business lines.

o Greeting visitors professionally.

o Maintain confidentiality and adhere to HIPAA compliance standards.

o Comply with company policies and maintain a clean office space.

o Act as a backup caregiver when needed.

o New client social medial posts

o Rando client visits when caregiver is scheduled

o Client outreach

o Client communication log ‘(random call log)

o Monitoring care notes for details regarding clients.

• Other duties as directed by Director of Operations.

Qualifications

- Degree in Social Work, Healthcare Administration, or related field.

OR

- Minimum of 2-3 years of experience in home care, case management, or healthcare

coordination.

- Strong understanding of healthcare regulations and standards.

- Excellent communication, organizational, and problem-solving skills.

- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.

- Ability to manage multiple priorities and work effectively in a dynamic environment. Work Schedule

• Regular Hours: Monday–Friday, 8:30 AM to 5:00 PM with a 30–minute lunch daily.

• Weekend Coverage: Every other weekend (Saturday and Sunday)

• This position may require occasional travel between the two office locations.

• Standard office hours with potential for on-call responsibilities during evenings and
