JOB DESCRIPTION
The RN Case Manager will provide comprehensive nursing case management and care coordination services to support patient safety, continuity of care, and access to appropriate healthcare services. The role involves working closely with patients, families, healthcare providers, and interdisciplinary team members to coordinate care and ensure patient needs are addressed effectively.
KEY RESPONSIBILITIES
- Assess patient needs and coordinate appropriate nursing and healthcare services.
- Develop, implement, and monitor individualized care plans.
- Coordinate care between patients, families, providers, and interdisciplinary teams.
- Monitor patient progress and identify changes in condition or care needs.
- Assist with transitions of care, referrals, follow-up appointments, and discharge planning.
- Communicate patient status and care updates with healthcare providers and team members.
- Review available clinical information to support appropriate care coordination.
- Maintain accurate and timely documentation in the electronic medical record.
- Follow established nursing, safety, compliance, and organizational procedures.
- Help identify barriers to care and coordinate resources to support patient access.
- Participate in interdisciplinary meetings and case-management activities.
- Provide patient and family education regarding care plans, services, and follow-up needs.
- Travel approximately 10% as required for the position.
- Support overall patient safety, quality of care, and continuity of services.
