Contracted Community Case Manager

The Contracted Community Case Manager position under MaineCare Section 13 OADS/DHHS offers an hourly contract opportunity at $65 per billable hour. This role focuses on managing a caseload of 15-20 clients, providing essential case management services, advocacy, and resource coordination within the community. The position requires frequent collaboration with external agencies and involves local travel for community visits. Ongoing mentorship and training are provided to support professional development.

Preferred Qualifications

• 1+ years experience in community case management
• Bachelor's degree in Social Work or related field (Required)
