RN Supervising Nurse – Home Health Agency (Houston, TX)

Optimal, is seeking a Registered Nurse Supervising Nurse to oversee clinical operations, support field staff, and ensure highquality patient care. This role provides leadership, conducts supervisory visits, and maintains compliance with HHSC and CMS regulations.

Key Responsibilities

• Complete initial assessments, care plans, and supervisory visits
• Oversee LVNs, CNAs, and attendants; provide training and support
• Review documentation for accuracy and compliance
• Coordinate care with physicians and interdisciplinary teams
• Participate in QAPI, audits, and regulatory readiness
• Ensure timely communication and continuity of care

Qualifications

• Current Texas RN license
• 2+ years nursing experience (home health preferred)
• Strong leadership and communication skills
• Knowledge of HCSSA, HHSC, and CMS regulations
• Reliable transportation
