Are you a detail-oriented and compassionate professional with a background in home health care? We are seeking an experienced
Intake Coordinator to join our growing team and be the first point of contact for patients entering our care.
? Key Responsibilities:
- Manage incoming referrals and initiate the intake process
- Verify insurance and obtain authorizations
- Communicate with physicians, hospitals, and case managers
- Enter patient and physician information into Kinnser (WellSky)
- Work closely with the Patient Care Coordinator to ensure staffing needs are met
- Coordinate with clinical staff to ensure timely admissions
- Ensure all documentation meets Medicare and agency standards
- Required to be ON-Call on a rotating shift.
Qualifications:
- 1–2 years of intake experience in home health care
- Strong working knowledge of AIDEN/AIDA and Kinnser/WellSky
- Excellent communication and organizational skills
- Ability to manage multiple tasks in a fast-paced environment
- Bilingual a plus!
