RN - Admissions (Part-Time)

Location: Newark, Ohio
Schedule: Part-Time | 10:00 AM–10:00 PM | Every 3rd Weekend + Rotating Holidays

Ohio’s Hospice is looking for a Part-Time Admission Care Liaison (Registered Nurse) to support patients and families as they transition to hospice care.

In this role, you'll help guide patients through the hospice admission process, complete clinical evaluations, collaborate with physicians and healthcare teams, and make sure patients and families understand the care and support available to them.

You'll work across hospitals, long-term care facilities, assisted living communities, and home settings, helping create a smooth transition to hospice services.

• Conduct comprehensive assessments of patients and families, addressing physical, emotional, psychosocial, and spiritual needs.

• Evaluate patients in hospitals, long-term care facilities, assisted living communities, and home settings.

• Educate patients and families about hospice services, eligibility, and available support.

• Help patients and families make informed decisions about hospice care.

• Collaborate with the interdisciplinary team to develop and initiate individualized plans of care.

• Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.

• Provide ongoing assessments, symptom management, and care coordination for patients in contracted facilities.

• Help coordinate transitions between hospitals, facilities, and home settings.

• Partner with physicians, hospital personnel, facility staff, and other care teams to support continuity of care and timely discharge planning.

• Assist with organizational and patient care initiatives across departments as needed.

• Participate in quality improvement activities focused on patient outcomes and service excellence.

• Mentor and orient newly hired staff while promoting teamwork and professional development.

• Graduate of an accredited nursing school.

• Current Registered Nurse (RN) license in Ohio with no board actions.

• Current CPR certification.

• Computer skills sufficient for accurate clinical documentation.

• Valid Ohio driver's license and acceptable driving history.

• Current automobile insurance meeting state minimum requirements.

• Ability to travel as needed and safely drive in varying weather and road conditions.

• Ability to successfully complete a background investigation and 10-panel drug screening.

• Two years of acute care nursing experience.

• CHPN (Certified Hospice and Palliative Nurse) certification.
