Job Details:
- Hours: Approximately 20 hours per week.
- Pay Rate: $17 per hour.
- Flexibility: Days and hours are flexible. Client would like evening shifts. Client is open to the caregiver making his/her own schedule.
Overview:
Attentive Heart of Care is seeking a compassionate, energetic, and experienced independent Direct Support Provider (DSP) to provide companionship to a 46-year-old male.
This contract opportunity is vital for supporting the client's health, safety, and independence within the community environments. As an independent contractor, you will deliver respectful, person-centered services aimed at enhancing the client's overall quality of life.
Responsibilities:
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance.
- Monitor client condition and promptly report changes in health status, behavior, or safety concerns.
- Support community outings and activities to promote independence, inclusion, and social integration.
- Prepare nutritious meals and snacks based on dietary needs and client preferences.
- Provide medication reminders and support self-administration as permitted by the care plan and agency policy.
- Maintain a clean and safe environment through light housekeeping and sanitation tasks.
- Document services provided accurately, including progress notes and activity logs, in compliance with HIPAA and agency requirements.
- Follow all safety procedures, including CPR/First Aid protocols and incident reporting.

Qualifications/Required Documentation:
- High school or equivalent (Preferred).
- Caregiving: 1 year (Preferred).
- US work authorization. (U.S. passport or U.S. passport card. Form I-551, Permanent Resident Card or Alien Registration Receipt Card (this is commonly called a Green Card.) (Preferred).
- CNA or HHA (40 Hours) Certificate (Accepted).
- Valid Driver's License & Social Security Card (Required).
- CPR/AED/First Aid Certification (Required).
- Auto registration & proof of Auto Insurance (Preferred).
- Liability Insurance (CM&F rate is $69.00/yearly) (Required).
- CEU certificates: HIV/AIDs, Zero Tolerance, HIPPA, Direct Core Competency (I will assist with this) (Required).
- Void check or equivalent (for bi-weekly direct deposit).
- Level 2 and local background check. (Preferred).
- 2 professional references. (Required).
Job Type: Part-time.
Pay: $17 per hour.
