Position: Per Diem / PRN
Position Overview
Countryside Health Care Center is seeking a compassionate and dedicated Per Diem Speech-Language Pathologist (SLP) to join our in-house subacute rehabilitation team. The SLP will provide comprehensive speech, language, cognitive, and swallowing services to residents recovering from illness, injury, surgery, or other medical conditions.
This position offers the opportunity to work collaboratively with an interdisciplinary rehabilitation team while helping residents improve their communication, swallowing safety, cognition, and overall quality of life.
Responsibilities
- Evaluate residents' speech, language, cognitive-communication, and swallowing needs.
- Develop individualized treatment plans based on clinical findings and resident goals.
- Provide skilled speech-language and swallowing therapy in accordance with established plans of care.
- Monitor resident progress and modify treatment approaches as appropriate.
- Complete accurate and timely clinical documentation.
- Collaborate with nursing, therapy, dietary, medical, and other interdisciplinary team members.
- Provide education to residents, families, and caregivers regarding communication strategies and swallowing safety.
- Participate in care planning and interdisciplinary meetings as appropriate.
- Follow all applicable professional, regulatory, and facility requirements.
Qualifications
- Master's degree in Speech-Language Pathology from an accredited program.
- Current Georgia Speech-Language Pathology license or eligibility for licensure.
- Current ASHA certification (CCC-SLP) preferred/required as applicable.
- Experience in skilled nursing, subacute rehabilitation, long-term care, or a related setting is preferred.
- Strong clinical, communication, organizational, and documentation skills.
- Ability to work effectively as part of an interdisciplinary team.
