Position Overview
The Home Health Clinical Referral Coordinator is responsible for managing and processing referrals for home health services from referral through admission or resumption of care.
This role serves as a primary point of contact throughout the referral process, ensuring all required documentation and information is collected, accurate, and complete. The position requires a combination of clinical knowledge, referral/intake experience, technical skills, and strong communication to support timely and compliant admissions.
Quality, accuracy, and timeliness are critical to success in this role.
Key Responsibilities
- Review, process, and verify referral documentation to ensure all required elements are complete.
- Coordinate with referral sources, patients/families, and internal teams to obtain necessary documentation and information.
- Manage referrals through the admission and resumption-of-care process.
- Maintain knowledge of program referral capacity and admission targets.
- Collaborate with program teams to meet daily, weekly, and monthly admission goals.
- Support clinical decision-making related to referral and eligibility determinations.
- Provide guidance, coaching, and support to clinical and non-clinical team members.
- Utilize referral portals to manage and process incoming referrals.
- Obtain orders and other documentation required for admission or resumption of care.
- Maintain accurate and current patient records throughout the referral process.
- Ensure compliance with home health policies, HIPAA, and applicable federal and state regulations.
- Participate in quality improvement initiatives, team meetings, training sessions, and daily stand-up meetings.
- Prepare reports and data as requested.
- Organize and prioritize large volumes of information while maintaining accuracy and efficiency.
- Provide exceptional customer service to patients, families, and referral sources.
- Triage incoming calls and inquiries to the appropriate team member.
- Provide information regarding home health services to patients and families as needed.
- Capture required information throughout the referral and intake process.
- Utilize established best practices to support an efficient referral-to-admission process.
- Maintain confidentiality of patient and organizational information.
- Field Support: May be required to work in the field to support clinical needs, which may include carrying a patient caseload.
- Perform other duties as assigned.
Required Qualifications
- High school diploma or GED required.
- 2-3 years of related home health referral or intake experience.
- Strong organizational, communication, and customer service skills.
- Ability to manage multiple referrals and prioritize time-sensitive information.
- Working knowledge of healthcare documentation and regulatory requirements.
Preferred Qualifications
- Associate or Bachelor's degree preferred.
- Experience with:
- Health insurance verification
- Prior authorizations
- Healthcare referrals
- Medicare/Medicaid billing and reimbursement
- Referral and intake portals
- Home health clinical or referral coordination experience.
