Job Description
Family Ark providescase managementservices to youth and families in southern Indiana. TheCommunity-BasedCase Managerprovidescase management servicesin the home and collaborates with team members, schools, families and other professionals to meet eachfamily'sspecific goals and needs. This individual works with DCS-involved familieswho have had a substantiated incident of abuse and/or neglect.

Key Responsibilities Include
- Facilitates case management services primarily in the home of the client.
- Maintains all client case records by documenting client contacts, as well as contacts with the Department of Children's Services, schools, parents, and other professionals.
- Develops treatment plans, revises treatment plans and writes and submits monthly reports to the Department of Children's Services.
- Participates in case conferences and court proceedings as necessary for client care.
- Participates in a 24-hour on-call rotation and provides support as needed.
- Other duties as assigned.
Minimum Requirements
- Bachelor's degree in social work, psychology, sociology, or a directly related human service field preferred.
- One year work or internship experience in the social service field.
- Strong verbal and written communication skills.
- Ability to interact effectively with people from diverse social and economic backgrounds
