CareBuilders at Home isseekinga compassionate and experienced Registered Nurse (RN) to conduct in-home assessments for clients who require support services in the home. We proudly serve the Collin County area with a team of highly trained and dedicatedstaff. Duties

• Conduct Onboarding in home assessments of clients seeking support services at home

• Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medicationreminders, community transportation, respite care, etc.

• Onboard new clients and periodically reassess client's progress with thein-homecare services

• Educate patients and their families on non-medicalin-homecare services and scheduling

• Utilizeelectronic medical record systems (AxisCare) to accurately document patient information

• Scheduleinitialcaregiver visits

Requirements:

• Pass drug screening

• Valid Driver's License

• Own Reliable Transportationto travel throughoutProsper and surrounding areas

• Proof of Car Insurance

• Social Security Card or Birth Certificate

• Negative TB skin test

• Provide three work related references
