Department: Resident Care Services 3p-11p

Reports to: Associate Director of Health and Wellness, Nurse Director of Health and Wellness & Administrator

Description of Position:

Coordinates resident care related to medications by working with all departments, the medical community, families and administrative staff to provide for resident needs with continuity and an adherence to the scope of practice and licensure for the community. Provides complete supervision of the medicationroom, pass techniques, documentationand supervision and provision of care related to medication in the community.

Responsibilities of the MedicationAide:

1.Resident charts. Keeping documentationcurrent (Community forms, licensing documentation, physicianorders, incident reports., etc.)

2.Communicate resident status changes to Lead Med Aide or Health and Wellness Director.

3.Ensure all medicationdocumentationis current and correct in E-MAR before administering.

4.Ensure medicationcart is completely stocked with all required continuous, PRN, Over-the-Counter (OTC), and othermedications as ordered by the physician.

5.Coordinate medication orders and deliveries with pharmacies; reordering medications on a weekly basis and as needed. This means utilizing the E-MAR to reorder as well as calling the pharmacy or family as needed to ensure there is always a supply of ordered medications at Maple Ridge Manor.

6.Communicate with physicians and other healthcare providers as needed, including reporting falls, injuries, and/or changes in resident status. Communication may be via verbal in person, fax, or phone and should be communicated to Lead Med Aide as well as Director of Health and Wellness.

7.Monitor Psychotropic med use is congruent with physicianorders and ensuring resident behaviors actually warrant the use of medication. This should be charted in the resident’s E-MAR so that the Director of Health and Wellness can monitor.

8.Control medicationroom access and key assignment, keys should be signed out at the beginning of every shift and signed back in at the end. Medication room door should always be shut when not in use.

9.Pour, pass, and assist with administration of medications in accordance with state regulations. Provide mentorship for new Medication Aides.

10. Coordinate physicianand other medical appointments. Notify Lead Aide and/or Health and Wellness Director to ensure any needs are communicated at time of appointment.

11. Read all communication notes regarding the community between the Caregiver shifts. Report any urgent communication to Lead Med Aide, Director of Health and Wellness.

12. Other duties as assigned.
