Job Summary:

Responsibilities:

• Conduct initial and comprehensive resident assessments, as well as accurate and timely updates, to ensure that all care plans meet the resident's needs and preferences

• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary care team

• Coordinate and communicate with healthcare providers, families, and other stakeholders to ensure continuity of care

• Provide hands-on nursing care, including administering medications, treatments, and therapies, as well as assisting with daily living activities

• Monitor and manage resident health status, identifying and reporting any changes or concerns to the charge nurse or physician

• Participate in quality improvement initiatives and maintain accurate records of resident care and services

Requirements:

• Current RN licensure in the state of Texas

• Strong assessment and problem-solving skills, with the ability to work independently and as part of a team

• Excellent communication and interpersonal skills, with the ability to effectively interact with residents, families, and healthcare providers
