Service Coordinator

Company: TruCaring®
Location: Fort wayne (Fort Wayne)
Salary per month: $2500.00 per month
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Status: Full-Time
Reports To: program director

Primary Responsibilities

  • Manage an assigned caseload
  • Communicate with clients and families
  • Coordinate with Regional Centers
  • Track IPPs and service authorizations
  • Conduct virtual and in-person assessments
  • Maintain case documentation and monitor services
  • Participate in IRP and interdisciplinary meetings
  • Identify service gaps and changing client needs
  • Coordinate with internal staff and operations teams
  • Escalate complex issues to the Director
Preferred Qualifications

  • Indiana Regional Center Service Coordinator experience
  • Experience supporting individuals with intellectual and developmental disabilities
  • Strong case management and organizational skills
  • Excellent communication and documentation abilities
  • Knowledge of IRPs, service authorizations, and person-centered planning
  • Ability to work independently and recognize when issues require escalation
  • Reliable transportation for occasional client visits
  • Must hold valid (current within one year of date of hire) or else complete a DOJ and FBI fingerprint and criminal clearance and a health screening including a TB test.
  • Strong interpersonal, communication, leadership skills.
  • Computer skills required.
  • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
Competitive Advantages

Candidates with direct Indiana Regional Center Service Coordinator experience will receive priority consideration and may qualify for compensation at the higher end of the salary range.
