Assist residents with personal care, nutrition, hygiene, and meal planning as needed.
Promote resident health and safety while providing education regarding medical conditions and medications.
Administer medications, including insulin and tube feeding when required by residents' treatment plans, in accordance with New York State AMAP regulations and applicable policies.
Assist residents with scheduling medical appointments and advocate for them during medical visits.
Ensure medical follow-ups are documented and communicated appropriately to the Registered Nurse and Residence Manager.
Provide first aid and emergency medical treatment within the scope of training and applicable requirements.
Track and report medical and medication updates, behavioral supports, daily goal implementation, and other required information.
Complete monthly medical and medication audits in collaboration with the Registered Nurse.
Attend monthly staff meetings and bi-weekly Interdisciplinary Team (IDT) meetings.
Participate in residents' annual Life Plan meetings.
Serve as a positive role model and promote healthy, respectful interactions.
Participate in required training related to incident prevention, response, and documentation.
Follow all applicable regulatory and agency policies and procedures for incident management and reporting.
Promptly communicate and document incidents and health-related concerns according to established procedures.
Accurately document services, medical appointments, medications, and relevant health information in the electronic health record system.
Maintain medical record binders for assigned residents.
Maintain residents' "To-Go" books for medical appointments and emergency or urgent-care visits.
Complete daily service delivery notes and other required documentation accurately and on time.
Maintain confidentiality and ensure records are handled in accordance with organizational policies and applicable regulations.
Communicate effectively and professionally with residents, families, coworkers, healthcare professionals, day habilitation programs, care coordinators, and other members of the support team.
Communicate medical needs, appointments, changes in condition, and other relevant information to appropriate team members.
Notify families of significant changes in residents' conditions, including urgent care visits and hospitalizations, as required.
Collaborate with the RN, Residence Manager, and interdisciplinary team to promote continuity of care.
Complete all required annual and recommended training.
Maintain required certifications, including AMAP, CPR, and other applicable credentials.
Accept assignments and perform additional duties as directed by the Residence Manager or designee.
Required:
High school diploma or GED.
Minimum of 2 years of experience working with individuals with intellectual and developmental disabilities (I/DD).
Current AMAP certification.
Knowledge of OPWDD medical and medication regulations.
Strong written and verbal communication skills.
Familiarity with electronic health records and electronic documentation systems.
Valid New York State Driver's License.
Ability to pass required background checks.
Ability to complete and maintain required certifications and trainings.
Smartphone required.
Must have completed the orientation period of 3–6 months in the current position, where applicable.
Must be in good standing in the current position, where applicable.
Must be current with all required mandatory trainings.
Ability to lift up to 50 pounds.
Ability to stand, bend, lift, and move throughout the workday.
Ability to provide physical assistance to residents when required and within applicable training and safety guidelines.
East Meadow and Massapequa, NY
The schedule will be shared between both residential locations.
