Howard University Hospital is seeking a compassionate and resourceful Social Worker to join our Case Management team. This position plays an essential role in supporting patients and families throughout the continuum of care by identifying psychosocial needs, addressing barriers to discharge, and coordinating the resources necessary to support safe and timely transitions from the hospital.
The Social Worker conducts comprehensive psychosocial assessments, coordinates inpatient discharge planning, collaborates closely with the interdisciplinary care team, and assists patients and families with post-acute care needs. The role also provides crisis intervention, emotional support, resource information, patient and family case conferences, and assistance with ethics consultations as appropriate.
Key ResponsibilitiesThe ideal candidate will have strong clinical judgment, communication, care coordination, and problem-solving skills, with the ability to collaborate effectively across an interdisciplinary healthcare environment.
Required QualificationsThis position primarily supports daytime operations, with evening, night, weekend, and holiday coverage required as needed based on departmental and patient care needs.
Compensation & Union StatusThis is a full-time, non-exempt position covered under the DC Nurses Association (DCNA) collective bargaining agreement. Pay range: $35.60 – $57.41/hr, commensurate with experience and qualifications.
