RN – Case Management | Travel Contract


Unit: Care Coordination

Position: RN – Case Management
Contract: 13 Weeks | 38 Hrs/Week | 49.5-Hour Shifts
Shift: Days | 7:00 AM–5:00 PM or 8:00 AM–6:00 PM

Requirements

  • Active New York RN License in hand
  • BSN required
  • Minimum 3 years of acute care hospital experience
  • Recent 3 years of Discharge Care Planning (DCP) experience
  • Experience in:
    • RN Case Management
    • Care Coordination
    • Discharge Planning

Schedule Details

  • On-Call: No
  • Weekend: Every other Sunday / approximately 2 weekends per month
