Case ManagerASHLIN Management Group is seeking a Case Manager to join our team.Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills and/or abilities.

Education/Experience

  • Bachelor’s degree in social work, psychology, sociology, counseling, or relatedservice/scienceorhealthcarerelateddisciplinesandoneyearofexperienceprovidingcasemanagement services (internship experience applicable). At least two years of workexperience in a social service and/or human service field will be considered in lieu of adegree. Certification and/or licensure in relevant discipline (for example, certified addictionscounselor)may also substitute foreducational requirements.
  • Bilingualapplicantswillberecruitedtoensurethatculturallycompetentandlinguisticallyappropriateservices areprovidedtoindividuals andfamilies.
  • Abilitytoworkwithclientsinacaringandrespectfulmannerandwithunderstandingofandconsiderationfor their unique circumstances
  • Ability to develop, evaluate, and implement a case management plan meeting allmilestones.Abilitytoidentifyresourcesandservicesandlinkclientsasnecessary

Job Responsibilities
  • Coordinate all services and help ensure that the client is making meaningful progresstowardincreasing earnings and securingstable housing
  • Introduce clients to the process, review the - assessments, develop and amend HousingStabilization Plans and DIRPs, help clients overcome barriers to housing stability and self-sufficiency.
  • Developtrustingrelationshipswithclientsassignedtothemandmonitorclients'progressonhousingstabilityjourney
  • Responsible for conducting outreach to referred clients, engage clients in 2Gen activities,re-engagingthoseclientswho startedtoparticipateand subsequentlystopped
  • Responsible for the engagement of clients and should identify goals and activitiesappropriateforhousingstability, barrierremovaland career-pathways.
  • ResponsibleforcoordinatingwithTEPproviderstoidentifyappropriateeducationandoccupationaltrainingprogramsinadditiontoemploymentopportunitiesforfamilies
  • Meetwithassignedclientsaswellasmaintaincasenotesonallclientcontacts
