We are seeking a dedicated and compassionate Community Paramedic to provide in-home and community-based healthcare services. This role involves assessing patients' health status, managing chronic conditions, and addressing social determinants of health to improve overall well-being. The Community Paramedic will serve as a vital link between patients and community resources, ensuring comprehensive care tailored to individual needs. Responsibilities

  • Conduct in-home assessments to evaluate patients’ health conditions, social needs, and living environments.
  • Manage chronic conditions through patient education, monitoring, and coordination with healthcare providers.
  • Provide care tailored to where patients live, ensuring accessibility and comfort during interactions.
  • Address social and basic needs by identifying social determinants of health that impact patient outcomes.
  • Connect patients with appropriate community resources such as housing assistance, food programs, mental health services, or transportation support.
  • Collaborate with healthcare teams to develop personalized care plans that promote health stability and prevent unnecessary hospital visits.
  • Document patient interactions accurately and maintain detailed records of assessments and interventions.
  • Educate patients on health management strategies and promote adherence to treatment plans.
  • Serve as an advocate for underserved populations by ensuring they receive comprehensive, culturally sensitive care.

Skills

  • Strong understanding of chronic disease management and preventative care strategies.
  • Excellent assessment skills with the ability to evaluate social determinants of health effectively.
  • Compassionate communication skills for engaging diverse patient populations in their homes or community settings.
  • Ability to connect patients with community resources and navigate social services systems.
  • Knowledge of local healthcare resources, social services, and support networks.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.

License/Certification - one of the following is required:

  • NREMT EMT Certification
  • NREMT AEMT Certification
  • NREMT Paramedic Certification
  • IBSC CP-C is required upon application or within 3 months of hire

Shift availability:

  • Day Shift
  • Night Shift
  • Overnight Shift
  • 2 Weekend Days are required per 2-week pay period
