Pay Rate: $50 – $55/hr

Position Overview
We are seeking an experienced RN Case Manager to provide case management, care coordination, discharge planning, and transition-of-care support across assigned campuses.
Required Qualifications
Minimum 2 years of Case Management experience.
Current RN license required and verifiable through Nursys.
BLS certification required.
Degree accreditation must be listed on the candidate profile.
Willingness to float to sister campuses as needed.
Travelers may be required to float to other sister campuses.
Must meet the 50-mile radius requirement from the facility to permanent address.
Strong communication and teamwork skills with the ability to function as part of the core team.
Firm start date required.
Care Management certification such as ACM or CCM preferred.
Responsibilities
Provide professional RN case management and care coordination.
Assess patient needs and coordinate appropriate services throughout the continuum of care.
Collaborate with physicians, nursing staff, social workers, and other members of the healthcare team.
Support appropriate discharge planning and transitions of care.
Coordinate resources and services based on patient needs.
Float to sister campuses as required.
Function as part of the core team and follow facility workflows, policies, and expectations.
