Addison Group is partnering with a mission-driven healthcare organization seeking a Nurse Case Manager to support individuals with intellectual disabilities and complex healthcare needs in residential and adult day program settings.

This is an opportunity for a compassionate nursing professional to make a meaningful impact by coordinating care, supporting health and safety, and working closely with individuals, families, healthcare providers, and interdisciplinary teams.

Job Description

The Nurse Case Manager will oversee and coordinate healthcare needs for individuals receiving residential and community-based services. This position will develop and monitor individualized care plans, coordinate medical services, maintain accurate documentation, and help ensure individuals receive safe, consistent, and appropriate care.

The ideal candidate is an organized and compassionate nursing professional who is comfortable collaborating with families, providers, direct-care staff, and other healthcare professionals.

Key Responsibilities

• Coordinate individualized healthcare plans and collaborate with interdisciplinary teams to support each individual's medical and personal needs.
• Monitor individuals' health through regular assessments and residential visits, identifying changes or concerns and coordinating appropriate follow-up.
• Serve as a liaison between individuals, families, physicians, healthcare providers, and program staff to ensure continuity of care.
• Manage medications, including pharmacy coordination, refills, prior authorizations, medication reviews, and oversight of administration procedures.
• Coordinate medical appointments, physician orders, referrals, and follow-up care to ensure timely completion of healthcare needs.
• Maintain accurate medical records, care documentation, and health information while ensuring compliance with applicable regulations and policies.
• Participate in care meetings, admissions, discharges, and staff training while providing nursing guidance and medical expertise.

Qualifications

• Active RN or LPN license in Pennsylvania with at least 2-5 years of clinical experience in acute or long-term care.
• Experience supporting individuals with intellectual or developmental disabilities is strongly preferred.
• Familiarity with CareLogic is preferred.
• Knowledge of medication administration, healthcare documentation, and regulatory compliance.
• Strong communication, organization, time management, and problem-solving skills with the ability to manage multiple priorities.
• First Aid/CPR certification preferred, with a commitment to maintaining confidentiality and accurate records.

Work Environment & Schedule

• Monday-Friday 8:00AM-4:30PM
• Occasional travel to assigned residential locations may be required.
