We’re hiring an experienced RN Case Manager for a hospital-based role in Glendale, Arizona. This position focuses heavily on discharge planning and coordinating post-acute care to ensure safe, efficient patient transitions.

• Conduct initial case management assessments

• Develop and coordinate discharge (DC) plans with patients, families, and care teams

• Arrange post-acute services based on patient preferences and needs

• Collaborate with physicians, nurses, and interdisciplinary teams

• Ensure compliance with hospital policies and regulatory standards

• Maintain accurate and timely documentation

• RN License – State of ArizonaOR Licensed Master’s Level Social Worker in AZ

• Minimum 5 years Case Management experience in an acute care hospital

• Strong experience with discharge planning and post-acute placement

• Excellent communication, coordination, and problem-solving skills
