What You’ll Do:
Coordinate patient care plans with physicians and multidisciplinary teams
Support discharge planning and continuity of care services
Advocate for patients and families while optimizing healthcare resources
Monitor care progression, length of stay, and patient outcomes
What You’ll Bring:
Active California RN license required
Case management experience preferred
