RYZE on the Avenue is seeking an experienced and dedicated Duet Program Manager to lead and oversee the implementation and day-to-day operations of our Duet Program. The Duet Program provides specialized, trauma-informed, patient-centered care for individuals with complex medical needs and behavioral or psychosocial health challenges.
The DUET Program Manager will coordinate an interdisciplinary team to ensure residents receive comprehensive medical, behavioral health, psychosocial, and rehabilitative services. This role will be responsible for program implementation, staff education, performance monitoring, quality outcomes, appropriate admissions, and successful transitions back to the community.
Duties and Responsibilities
- Oversee the day-to-day operations and implementation of the DUET Program
- Ensure the program is delivered in accordance with established clinical, behavioral health, regulatory, and operational standards
- Coordinate with nursing, medical, psychiatric, social services, therapy, dietary, and other departments to provide integrated resident-centered care
- Review and assist with appropriate DUET admissions and ensure residents meet program criteria
- Coordinate comprehensive assessments and individualized treatment plans addressing both medical and psychosocial needs
- Monitor resident progress toward clinical, behavioral, functional, and community reintegration goals
- Facilitate interdisciplinary DUET meetings, care planning, and case reviews
- Develop and coordinate ongoing education and training for staff regarding the DUET Program, trauma-informed care, behavioral health, and disease management
- Monitor program outcomes, including hospitalizations, readmissions, length of stay, discharge destination, resident satisfaction, functional outcomes, and achievement of treatment goals
- Identify opportunities for program improvement and implement corrective or performance improvement initiatives
- Work closely with the Medical Director, attending physicians, psychiatric providers, Director of Nursing, Social Services, and other clinical leaders
- Support the development and implementation of psychosocial programming, community reintegration, life skills, and supportive services
- Assist with discharge planning and ensure appropriate community resources, home health services, DME, and other supports are arranged
- Maintain relationships with hospitals, community providers, behavioral health organizations, and other referral partners
- Participate in marketing and outreach efforts to promote the DUET Program and develop referral relationships
- Ensure accurate documentation and compliance with applicable federal and state regulations
- Monitor adherence to DUET policies, procedures, protocols, and behavioral program requirements
- Serve as a resource and advocate for DUET residents, families, and staff
- Participate in quality assurance and performance improvement activities related to the program
- Perform other duties as assigned
Qualifications
- Bachelor’s degree in Nursing, Social Work, Psychology, Healthcare Administration, or a related field required
- Registered Nurse (RN) preferred, with strong clinical experience in skilled nursing, post-acute care, or behavioral health
- Previous experience working with individuals with co-occurring medical and behavioral health needs strongly preferred
- Experience in skilled nursing, post-acute care, behavioral health, psychiatric rehabilitation, or a similar setting
- Previous program management, supervisory, or leadership experience preferred
- Strong understanding of trauma-informed care, behavioral health, psychosocial rehabilitation, and community reintegration
- Strong knowledge of interdisciplinary care planning and discharge planning
- Excellent leadership, communication, organizational, and problem-solving skills
- Ability to coordinate multiple departments and manage complex resident needs
- Strong documentation and computer skills
- Ability to develop relationships with residents, families, healthcare providers, hospitals, and community organizations
