DIRECTOR OF CARE MANAGEMENT – PERMANENT

Des Moines, IA
$120,000–$170,000/year
Monday–Friday | 8:00 AM–5:00 PM
On-Site | 24/7 Leadership Responsibility | On-Call Required

We are seeking a strategic and collaborative Director of Care Management to provide operational and strategic leadership for hospital and ambulatory care management programs. This role leads multidisciplinary teams and drives initiatives that improve patient outcomes, care coordination, patient flow, and resource utilization.

Requirements:

  • BSN, BSW, or equivalent healthcare management degree
  • Current RN or Licensed Social Worker license in good standing
  • 3–5 years of progressive leadership experience in Care Management, Population Health, or Healthcare Management
  • Iowa license required; Compact RN license accepted
  • Strong leadership, communication, and strategic planning skills
