A travel pharmacist opportunity is available at a 50-bed hospital in Northern Wisconsin, starting as soon as possible for a 13-week or longer assignment. This contract position offers a schedule of 12-hour day shifts on a rotating 7 days on, 7 days off basis.
Responsibilities include:
- Reviewing medication orders to assess drug therapy appropriateness, dose accuracy, dosage form, regimen, therapeutic duplication, and potential drug interactions
- Providing pharmacokinetic and anticoagulation dosing support
- Participating in clinical pharmacy services such as medication reconciliation and discharge medication education
- Overseeing pharmaceutical compounding activities while ensuring compliance with USP guidelines, specifically USP 797 and USP 800
- Utilizing the EPIC electronic health record system for documentation and communication
Qualifications and desired experience:
- Licensed pharmacist with active credentials to practice
- Strong knowledge of clinical pharmacy practices and medication safety standards
- Experience with pharmacokinetics and anticoagulation management
- Familiarity with pharmaceutical compounding requirements and USP regulatory standards
- Proficiency in EPIC or similar electronic medical record systems
