Pharmacist in Sun Prairie, WI

Provide pharmaceutical care as a licensed pharmacist in an acute inpatient rehabilitation setting, focusing on patients recovering from stroke, brain and spinal-cord injuries, amputations, complex orthopedic injuries, and other rehabilitative needs. This contract role is based in Sun Prairie, WI, with work performed at a specialized 50-bed rehabilitation hospital located in Madison, WI.

Qualifications and Experience:

• Pharmacist licensed in Wisconsin (WI)
• Minimum of 1 year experience in acute care or long-term acute care (LTAC) pharmacy practice
• Familiarity with EPIC electronic health record system and Omnicell medication management technology
• Local candidates preferred and accepted

Location:

• Sun Prairie, WI (work at the Madison facility of a rehabilitation hospital)

Responsibilities:

• Dispense and manage medications for complex rehabilitation patients
• Collaborate with multidisciplinary healthcare teams to optimize pharmacotherapy
• Utilize EPIC and Omnicell IT systems to document and monitor medication therapies
• Ensure compliance with regulatory standards and hospital policies
• Support patient safety and quality of care initiatives specific to inpatient rehabilitation
