Pay Rate
Local: $50 – $54/hour
Traveler: $55 – $60/hour

Position Overview
We are seeking an experienced RN Case Manager to coordinate discharge planning, continued-stay authorizations, and patient transitions in an inpatient rehabilitation setting.
Required Qualifications
Active Texas RN License.
BLS certification.
Minimum 2 years of experience in acute care case management, inpatient rehabilitation case management, or utilization review.
Strong knowledge of discharge planning and continued-stay authorization processes.
Interview required.
Ability to work Monday through Friday, 8:00 AM – 5:00 PM, with flexibility for weekend and after-hours admissions.
Preferred Qualifications
Experience with insurance payer portals.
Experience with WellSky EMR.
Responsibilities
Manage case management activities for patients in an Inpatient Rehabilitation Facility (IRF).
Complete initial discharge planning assessments within 2 days of admission.
Participate in the initial interdisciplinary team meeting by day four and weekly thereafter.
Coordinate discharge needs, including home health, outpatient therapy, durable medical equipment, dialysis arrangements, and post-discharge follow-ups at 72 hours and 30 days.
Complete continued-stay payer reviews and authorization processes.
Use payer portals, including Availity and Home & Community, for authorization reviews.
Re-issue the Important Message from Medicare (IMM) as required.
Coordinate with physicians, nursing staff, therapy teams, patients, families, insurers, and interdisciplinary team members.
Support timely patient transitions and discharge planning.
Maintain accurate case management documentation in WellSky.
Manage a fast-paced environment with an average patient length of stay of approximately 11.2 days.
Provide weekend and after-hours admission coverage as needed.
