Intake Coordinator

SNELLING is currently recruiting for an Intake Coordinator for a home health company in the Tyler, TX area. This is a full-time position. The Intake Coordinator manages the initial patient referral process, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care.

In this role, you will be responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient data into Electronic Medical Records (EMR). This position requires excellent computer and customer service skills, a strong work ethic, and the ability to work independently in a fast-paced environment.

Key Responsibilities:

• Receive and appropriately document new referrals in a timely manner.
• Verify each patient's payer source and maintain/track insurance authorizations and initial certifications.
• Coordinate care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provide effective communication to patients, staff members, other health care professionals, and referral sources.
• Respond to Agency and patient needs in a professional and creative manner.
• Participate in Agency-sponsored in-service training and the Agency's QAPI program.
• Promote the Agency philosophy and administrative policies to ensure quality of care.

Qualifications:

• Minimum one year of experience in referral intake preferred.
• Experience with third-party payers, Medicare, and Medicaid highly desirable.
• Medical field background preferred, including familiarity with medical terminology.
• Excellent computer and customer service skills.
• Valid Driver's License and Valid Auto Insurance.
• HomeCare, HomeBase experience preferred.
• Must pass a background check and drug screening.
