Deer Meadows Home Health is seeking a compassionate, motivated, and experienced Speech-Language Pathologist (SLP/CCC) to join our growing home health team!

Compensation & Pay Structure

We offer competitive compensation based on experience, visit type, and employment status.

Full-Time Compensation (If Applicable):

• Up to $110,000 annually, based on experience and qualifications

Per-Visit Compensation:

• Start of Care (SOC): $175 per visit

• Resumption of Care, Recertifications & OASIS Discharge: $100–$125 per visit

• Routine Speech Therapy Visits: $80–$90 per visit

• Meetings: $40 per meeting

Mileage Reimbursement:

• $0.58 per mile, reimbursed for eligible mileage after the first 20 miles of the workday

Compensation is based on experience, qualifications, employment status, and applicable visit types.

Key Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders

• Develop and implement individualized treatment plans based on patient needs and goals

• Provide speech therapy services in patients' homes, assisted living communities, independent living communities, and personal care settings

• Complete reassessments, progress notes, and discharge planning

• Educate patients, families, and caregivers on treatment strategies and home programs

• Collaborate with physicians, nursing staff, and interdisciplinary care teams

• Maintain accurate and timely documentation using Home Care Home Base (HCHB) EMR

• Independently manage patient caseloads, scheduling, and travel within the assigned territory

Qualifications & Requirements

• Master's or Doctorate degree in Speech-Language Pathology

• Active Pennsylvania Speech-Language Pathologist License – Required

• ASHA Certificate of Clinical Competence (CCC-SLP) – Required

• Prior Home Health experience – Required

• Experience with Home Care Home Base (HCHB) preferred

• Valid driver's license and reliable transportation

• Strong clinical assessment, communication, and organizational skills

• Ability to work independently and manage a flexible patient schedule
