Job Role:- MDS Coordinator
Location:Snow Hill, NC
Company is seeking an experienced MDS Coordinator (RN) to manage the Resident Assessment Instrument (RAI) process and ensure accurate, timely MDS assessments in a skilled nursing facility.
Key Responsibilities:
- Complete MDS assessments in compliance with federal and state regulations.
- Develop and update individualized resident care plans.
- Collaborate with nursing, therapy, and interdisciplinary teams.
- Ensure accurate clinical documentation and regulatory compliance.
- Monitor quality measures and support reimbursement and quality improvement initiatives.
- Educate staff on MDS guidelines and documentation standards.
Qualifications:
- Active RN license in good standing.
- Strong knowledge of MDS, RAI guidelines, and long-term care regulations.
- Experience in clinical assessments, care planning, and documentation.
- Excellent communication, organizational, and critical-thinking skills.
- Skilled nursing facility MDS experience and RAC-CT certification preferred.
