Weekend Home Health RN

Our client is seeking a compassionate and experienced RN to join their extraordinary care team. This full-time position involves providing management and delivery of patient care plans in Western PA and surrounding areas. The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience.

Key Responsibilities
Lead and collaborate to provide management and delivery of patient care plans
Support patients in Western PA and surrounding areas
Spend ~20% of the day driving to/from patient locations
Consistently meet critical deadlines for charting
Self-manage time and schedule

Schedule and Shift Details
Schedule (Including any on call/weekends): 8 units (roughly 8 patients) Saturday and Sunday, on-call Friday, Saturday, and Sunday from 5 PM-10 PM
