Fairview Rehab and Care is seeking a compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team in Philadelphia,PA.

As part of our collaborative rehabilitation team, you will provide personalized, resident-centered therapy and develop individualized treatment plans that support meaningful progress, improved function, and a higher quality of life. Key Responsibilities
- Evaluate residents’ mobility, strength, balance, and functional needs
- Develop and implement individualized physical therapy treatment plans
- Provide skilled therapy to reduce pain, restore function, and promote healing
- Monitor resident progress and adjust treatment plans as needed
- Educate residents, families, and caregivers
- Collaborate with the interdisciplinary care team
- Maintain accurate, timely, and compliant documentation
- Ensure services meet all professional, legal, and regulatory standards
Qualifications
- Active Physical Therapy license in PA, in good standing
- Skilled nursing or long-term care experience preferred
- Strong clinical, communication, and organizational skills
- Knowledge of the Net Health Electronic Documentation System preferred
