About the Role: Quality Care 4U Community Services in Pittsburgh, PA is looking for a passionate and dedicated Program Specialist to join our team! In this role, you'll play a vital part in delivering meaningful support and services to individuals in our community, making a real difference every day.
Responsibilities:- Develop, implement, and monitor individualized program plans for clients and participants
- Coordinate and deliver community-based services and support programs
- Maintain accurate and up-to-date client records, documentation, and progress notes
- Collaborate with interdisciplinary teams, families, and community partners to ensure quality care
- Monitor program outcomes and report on progress toward established goals
- Ensure compliance with regulatory standards, policies, and procedures
- Provide advocacy and resource referrals to clients and their support networks
- Collaborate with client and scheduling needs
Requirements:- Bachelor's degree in Social Work, Human Services, Psychology, or a related field
- Prior experience in community services, case management, or a related role preferred
- Strong knowledge of person-centered planning and community support services
- Excellent written and verbal communication skills
- Proficiency in documentation, recordkeeping, and case management software
- Compassionate, client-focused demeanor with strong problem-solving abilities
- Valid driver's license and reliable transportation preferred
